If you have selected to file an amendment as an attachment on the financing statement page of the application, you will now have the opportunity on this page to enter the extra filing information. The selections/entries on the page are all optional, but will determine what will appear on the filed Addendum to the UCC 1 Financing Statement. If you have questions on what you should select, the sample form with instructions is available to you by using the blue button at the top right on the page. When you are finished, click on Continue to proceed. Important: If you are presented with this page while filing and you did not intend to file an Addendum (UCCAd) form as an attachment to your Financing Statement, you will be able to remove this from your filing on the confirmation page before you file.
